  • Stall rate — what % of deals or projects have had no update or activity in 30+ days. Default benchmark: 22%.

Accountability Diffusion
Every decision travels through too many layers before anything happens. By the time it comes back, the window has closed — or no one remembers who was supposed to own it.
As organizations add management layers, decision speed and accountability decline.
How to fill this in:
  • Total annual payroll — combined salaries of everyone in the organization. Solo operators: your own annual revenue or owner draw.
  • Management layers — count levels from most junior individual contributor up to the top decision-maker (e.g. Staff → Lead → Manager → Director → CEO = 5 layers). Healthy: 4–5. Not applicable for solo operators or teams of 2.
  • Decision bottleneck — for teams under 3, estimate the % of decisions that require your personal approval before moving. Typical bottleneck drag: 12%.
